You must submit an itemized copy of a monthly bill that was issued in November or December 2025.

Before submitting, you must black out any sensitive personal information (such as full account numbers or SSN). However, to allow for proper verification, please ensure you keep the LAST 4 DIGITS of your account number clearly visible.

There is no limit to the number of unique bills you may submit, but each bill can only be submitted one time.
