A federal investigation into practices at the Veterans' Victory House in Walterboro, S.C. revealed that medications were being improperly administered. Now, the facility has to pay a daily fine of $515 until it is in "substantial compliance" with the provisions it violated.

Specifically, the investigation found that a physician's notes about how and when to administer medications were not followed by staff. The details of what medications and what effects the patient experienced as a result were not released in the report, according to The Post and Courier. However, the report did say that the staff member who administered the wrong medication has been fired.

This is not the first incident of violations at Veterans' Victory House. A 76-year-old patient was beaten to death by another patient with a cane in September 2008. The facility failed to properly report the incident.

The facility stands to lose its Medicare and Medicaid provider agreements if significant changes are not made.

The facility is overseen by the South Carolina Department of Mental Health.
