(WBBM NEWSRADIO) -- United Airlines and the FAA are trying to figure out how an emergency evacuation slide from a plane landing at O'Hare Airport Monday afternoon fell out of the plane and landed in a Northwest side neighborhood.

A United Airlines spokesman says ground crew members were going over Flight 12 after it landed from Zurich when they realized that the compartment that usually holds the slide was open and empty.

They then contacted the Federal Aviation Administration.

The Boeing 767-300 landed safely and normally at 12:35 p.m. with ten crew members and 150 passengers on board.

Police were called to an address on Chester Avenue south of Lawrence just after the plane landed.

Images on the scene show the slide on the street in front of a home. Airport workers collected the slide as part of the investigation.

No one on the ground was reported hurt.
