One man is dead after a possible fall at Sunday night's Phish concert at Chase Center, and another is "seriously injured" following a separate fall at the San Francisco venue.

In the second incident, one man fell on another and both were hospitalized with non-life-threatening injuries, the San Francisco Police Department said in a statement to KCBS Radio.

Golden State Warriors and Chase Center spokesperson Kimberly Veale said the organization is "working with local authorities to determine exactly what happened" at the band's second of two San Francisco shows this past weekend.

"Two guests attending last night's concert were seriously injured," Veale said in a statement to KCBS Radio. "The San Francisco Medical Examiner's Officer has informed us that, sadly, one of the individuals has passed away. We extend our heartfelt condolences to the guest’s loved ones."

Phish had not posted about either incident on its social media pages as of press time.

San Francisco police said that on-duty officers at the arena were informed a person needed medical assistance at around 8:55 p.m. on Sunday. An adult man "was suffering injuries from a possible fall," and he was pronounced dead after not responding to medical treatment.

Fifty minutes after on-duty officers were alerted to the first incident, police learned of another man who fell. The man fell on another concertgoer, both of whom were later hospitalized due to their injuries. Police said the man who fell required immediate medical attention from nearby medics.

Both incidents are under investigation. Police said they found "no evidence of foul play" in the man’s death, and they found "no evidence of any criminal violation" in the second incident.

The San Francisco Office of the Chief Medical Examiner is conducting an investigation into the man’s death, according to police.
