Multiple agencies are investigating after an 82-year-old woman was found dead outside her Ross Township personal care home.
Nancy Tyler lived at Mt. Assisi Place.
She was reported missing Sunday morning, and found dead on the property about an hour later.
The Allegheny County Medical Examiner ruled her cause of death heart disease, but her manner of death an accident.
Tyler's family is asking how she got out without notice in the first place.
The Department of Human Services says it's investigating, while Mt. Assisi Place says it cannot comment.
