The State Department of Labor and Industry is warning Pennsylvanians about signs of identity fraud on a tax document.
The DLI's Acting Secretary Jennifer Barrier says people who did not apply for unemployment benefits, but who get a 1099-G form saying they did receive unemployment payments need to fill out a fraud report online.
"It's really important for them to file the fraud form online so that we can begin an investigation and send that information to the IRS and not affect their tax filing."
The form is on the state's office of unemployment compensation website.
