A Mission Hills nursing home's license could be at risk after being found responsible for the deaths of two residents in less than three months.

In March, a resident at Ararat Nursing Facility known for being at risk of choking on food died in the hospital after she stopped breathing during a meal. A second woman died in June after falling from a lifting device. Staff was not present during either incident.

The L.A. Times reports that state public health investigators discovered violations that were “a substantial factor” in the two deaths. The nursing home was cited and fined $120,000 for each violation.

Ararat Nursing Facility is “in the process of appealing the citations in Superior Court,” representatives said in a statement to the L.A. Times.
