More than a year after a deadly plane crash in Addison, the NTSB has released a "factual summary" of its investigation although the final report won't be out for several months.
In June of last year, a twin-engine Beechcraft King Air crashed at Addison Airport, killing all ten people aboard.
The NTSB summary details their evidence but reaches no conclusions.
Just one minute after take-off, on the cockpit voice recorder, the co-pilot is heard telling the pilot: "you just lost your left engine."
Less than a minute after that, the plane slammed into a hangar.
The summary raises questions about the pilot, Howard Cassady, who apparently neglected to conduct the customary pre-flight check-list.
About an hour before the flight, the voice recorder shows Cassady spoke with someone about a problem with one engine known for burning oil.
