An audit of the Dallas Police Department has found that some forms related to domestic violence cases are missing, raising concerns about documentation practices.

While the audit concluded that Dallas police are generally following domestic violence protocols and effectively identifying high-risk victims, it also found gaps in paperwork. Specifically, some forms assessing a victim's needs — including proper shelter and risk level — were not completed.

Jan Langbein, CEO of Genesis Women's Shelter in Dallas, emphasized that addressing domestic violence requires more than just police and shelter involvement, given the widespread nature of the issue.

The audit made several recommendations to improve documentation, including ongoing training on proper procedures and ensuring that officers record when a victim declines to answer questions.
