PHILADELPHIA (KYW Newsradio) — If you have been to your doctor's office recently, you may have been asked to fill out several forms before the visit. The computerization of health care has allowed for larger patient records with far more information than when charts were handwritten.

The use of screening questionnaires is growing, but do they help you get better care? An editorial in the American Family Physicians suggests that it depends on how they are used.


The key to success is taking the information and having a conversation with the patient. It is all too easy to put the answers in a formula and make a decision, but it is the one-on-one conversation and follow-up that leads to the best care.
