The Philadelphia Eagles lead the league with 31 penalties, at the time of this writing, and have nine so far on Monday Night Football against the Dallas Cowboys. It hasn't been a pretty showing so far, and the penalties certainly haven't helped — though there's plenty of time left — and one of the usual suspects has contributed in the penalty column like several others.

His name is Derek Barnett, and he had a brutal unnecessary roughness penalty against the 49ers in Week 2 that head coach Nick Sirianni called "unacceptable" after the ugly loss. And in Week 3, when Barnett was whistled for an offsides penalty, Sirianni was voicing his displeasure once again — at least that's what it looked like if you read his lips from the sideline.
It's not true that it's "always" him but... yeah, it's him a pretty good amount. Barnett led the Eagles with four offsides penalties in 2019, when no other linemen had more than one call against them, and he also led the team with three unnecessary roughness calls (via The Football Database). He also led the team in offsides penalties in 2020, albeit with only two, and contributed another unnecessary roughness call there. He's on the board in both fouls early into 2021, and you have to wonder if Sirianni is already losing his patience with Barnett after just three weeks at the helm.
