
SAN FRANCISCO (KCBS RADIO) – Police are investigating the death of a woman at a Peninsula senior living facility who, along with two other residents, was mistakenly served dishwashing liquid instead of drinking juice.

Three homecare residents at Atria Park, an assisted living and memory care center in San Mateo, were hospitalized on Sunday after ingesting toxic chemicals, according to a release by police.
One of the victims, a 93-year-old woman, was pronounced dead while at a local hospital. KRON reported her identity as Eliza Murison Maxwell, a parent of eight children and grandmother to 20 others.
One of Maxwell's daughters, Marcia Cutchin, told the station her mother suffered from dementia and couldn't feed herself. She arrived at the hospital with "severe blistering of her mouth and throat and esophagus" and the family was told there was nothing they could do to treat the blistering.
In a statement provided to KCBS Radio, Atria Park confirmed that three of its residents were hospitalized "after mistakenly being served dishwashing liquid as drinking juice."
"We have been working with local authorities, who have informed us that one resident passed away," the statement read. "Our sincerest condolences are with the family. When this occurred, our staff immediately contacted authorities, and the residents were transported to the hospital for evaluation and treatment. We are conducting our own internal investigation, and the employees involved have been suspended until this investigation concludes."
San Mateo District Attorney Stephen Wagstaffe told KCBS Radio that the case was presented to his office on Monday, but it concluded based on the evidence that there was no criminal behavior.
The San Mateo Police Department is leading an active investigation into the incident. KCBS Radio reached out to the police department for additional information and has not received a response.
