A worker is dead after being crushed beneath a collapsing concrete burial vault at Restland Funeral Home in Dallas Monday afternoon.

Dallas Fire-Rescue says crews responded to the scene off Greenville Avenue and used heavy equipment to free the trapped worker.

Officials say the man was rushed to a nearby hospital with critical injuries but did not survive. Investigators are now working to determine what caused the vault to give way.

The Occupational Safety and Health Administration (OSHA) has been notified and is expected to review the incident. Restland Funeral Home, one of the largest cemeteries in North Texas, remained open while emergency crews cleared the scene.
