Buffalo, N.Y. (WBEN) - The Veterans Health Administration is sending letters to over 24,000 veterans in New York and New Jersey informing them of a mishap in which a limited amount of information may have been sent to another VA patient on notification postcards.
The VHA says no social security numbers or other identifying information were included in the notifications.
The postcards contained reminders for appointments scheduled between December 11th and December 15th of last year.
Any veterans affected by the error will receive a Privacy HIPPA Notification letter.
The VHA says the mistake was caused by a Xerox printing error.
Veterans who have questions or concerns may call 1-844-838-5433.
