A Dairy Queen location in Southwestern Pennsylvania was hit with several health violations after in inspection on Monday.

The PA Department of Agriculture’s Bureau of Food Safety says the Dairy Queen on West George Street in Carmichaels was hit with six violations following the inspection.

Ruled “out of compliance”, inspectors say they found a “black mold like substance” inside of an ice machine. The unit was shut down and the ice was thrown away.

Other issues include a “heavy build-up” of food inside a sandwich unit, old, splattered food inside a microwave, old, splattered food build-up in a cooler, area around lids for toppings with “heavy build-up of old encrusted food” and a busted plastic tray for hot hamburger storage.

Floors throughout the facility (kitchen, walk-in cooler, walk-in freezer) have heavy accumulation of dirt and other debris.

There were also other issues including a black mold substance in the caulking along the back of a three-bowl sink and no cold running water in the hand washing sink.

You can see the report and any updates to the restaurant’s status here. The Dept. of Agriculture says the facility has until May 11 to make corrections.
