For the second time in the span of a few days, the Orleans Parish Prison released an inmate who never should have been set free.

Now, the Orleans Parish Sheriff’s Office says that inmate has been caught and is once again behind bars.

According to a statement issued by the OPSO Monday night, jail officials notified the sheriff’s office about the improper release of Seth Butler. Butler, 19, has been in jail since August 2025 for violating his probation which stemmed from juvenile charges for which he was convicted in 2024.

“In February 2026, during the prior OPSO administration, OJJ emailed a detainer to OPSO Records to hold Butler for pick up by OJJ after the adult case was complete,” the OPSO’s statement said. “The emailed detainer was never added to Butler’s OPSO file or the Jail Management System.

“Butler pled guilty to his adult charges in Section A of Criminal District Court on May 29, 2026, and was given credit for time served and a suspended sentence by Judge Levine. The judge issued a release on the adult charges following the plea.”

Butler was released on May 30. The Office of Juvenile Justice notified the sheriff’s office of the error on Monday, leading to a multi-agency search for Butler. Sheriff’s deputies found Butler at his home and arrested him without incident.

Just three days prior to Butler’s erroneous release, jailers at the OPP mistakenly released another inmate, Dreion Williams, who was awaiting trial on battery and assault charges. Sheriff Susan Hutson attributed that release to “human error” and former Sheriff Susan Hutson implementing a new jail management system in the final few weeks of her term.
